Rabies Druid Build Guide
Overview
The Rabies Druid operates on a fascinating mechanic: infect one enemy with rabies in Werewolf form, and the disease spreads from monster to monster like a plague. Touch the first target, and the entire pack slowly drowns in poison damage over time — all without you needing to hit each one individually. The concept is genuinely powerful on paper and legitimately fun in practice. Well-geared, this build delivers some of the highest total poison damage-over-time in the game: a single bite can seed millions of poison damage across a pack.
The challenges are real: poison immunities are prevalent in Hell, and poison damage has a fixed duration (re-applying refreshes, doesn't stack). You need Lower Resist or a Reaper's Toll merc to crack immunities. With Plague runeword and Poison skill charms, Rabies becomes a legitimate C-tier Hell farmer capable of handling Baal waves, Chaos Sanctuary, and many density areas — slower than Wind Druid but uniquely satisfying.
Skill Allocation
20 pts — Rabies (primary damage + spread mechanic — increases poison damage and spread radius)
20 pts — Poison Creeper (key synergy — each point adds significant poison damage to Rabies)
20 pts — Werewolf (attack speed + life, enables Rabies and Feral Rage)
20 pts — Lycanthropy (more life, longer shapeshifting duration)
1 pt each — Feral Rage (life leech + run speed stacks), Fury (physical backup), Oak Sage, Grizzly, Cyclone Armor
Poison Creeper is the most critical synergy — without it maxed, Rabies damage is underwhelming in Hell. Remaining points after synergies go into Fury for physical damage against immune targets or into Oak Sage for more life.

The Reaper's Toll merc is the secret weapon — Decrepify procs frequently, slowing enemies and enabling your Fury backup attacks to hit harder. Keep a Lower Resist wand on weapon swap to manually crack poison immunities before the merc handles them.
Playstyle Tips
- Rabies spreads from the initial target to nearby monsters — bite the most central enemy in a pack for maximum spread.
- Poison damage over time is enormous: a fully geared Rabies Druid can push millions of poison damage per pack from a single bite.
- Re-applying Rabies to an already-infected target refreshes the duration but does NOT stack damage. Spread first, then move on.
- Use Feral Rage to build run speed stacks before engaging — then bite the target and kite while poison ticks.
- Fury provides reliable physical backup damage against poison-immune targets — don't neglect it entirely.
- Plague runeword's -enemy poison resistance stacks with your -resist from charms and mercenary for a huge damage multiplier.
- Trang-Oul's Claws and Belt are extremely cost-effective — huge poison bonuses for cheap items.
- Best farming spots: Baal Waves, Chaos Sanctuary clear, Flayer Jungle — anywhere monsters pack tightly enables maximum spread.
