Andariel, Maiden of Anguish — Boss Guide

By the DiabloBytes team · Updated May 2026

Andariel is one of the four Greater Lair Bosses in Lord of Hatred. The fight is a stacking-debuff endurance test — every mechanic you fail makes the next one hit harder. By sub-25% HP, accumulated stacks turn even a tanky build into a glass cannon. The reward is one of the best Mythic Unique loot tables in the game, plus Andariel\'s Loot Hoard on the Tormented variant.

⚡ TL;DR

  • Location: Hanged Man\'s Hall summoning altar, Kehjistan.
  • Summon cost: 1 Greater Lair Key.
  • Hardest mechanic: Effigy phase at 75% — burn 3 Effigies fast or the rotating blood wave wipes you.
  • Stacking debuff: Every avoided mechanic now matters in late phase. Play tight from open.
  • Reward: Greater-tier Mythic loot table (incl. Harlequin Crest), Azurewrath, class Uniques. Tormented = bonus Loot Hoard + Resplendent Spark.

Fight Mechanics

Phase Mechanic How to Handle
Open Dash attacks Andariel dashes across the platform every 8-12 seconds. Sidestep perpendicular to her movement vector — never run straight away from her.
Open Poison projectile arc Fans a wide arc of poison projectiles in front of her. Each hit applies a stacking poison DoT. Avoid by hugging her flank or dashing through the cone before it fully fans out.
Open Lightning balls Spawns slow-moving lightning orbs around the arena. Easy to avoid with normal movement awareness, but tracks if you stand still.
Open Ground Seals Persistent damage zones. Stepping into a Seal applies a stacking debuff that increases damage taken from her other attacks.
Phase 1 Transition Effigy spawn (~75% HP) Andariel flies up and summons 3 Effigies of Anguish around the arena. While they live, a rotating blood wave deals massive damage. Burn the Effigies before the wave catches you twice or the run ends.
50% HP Outer fire beams Statues spawn on the platform edge and channel fire beams toward the center. Move clockwise around the inner ring while continuing to dodge poison cones and lightning balls.
Sub-25% Stack mechanic peaks Damage taken from her abilities is multiplied by your accumulated debuff stacks. Players who got hit early in the fight die fastest in this phase. Play tight from the opening.

Drop Table

  • Mythic Unique pool Greater Lair Boss with access to the universal Mythic pool, including Harlequin Crest.
  • Azurewrath (Unique sword) Listed Andariel drop on current target-farm tables.
  • Class-specific Uniques Andariel rolls on the Greater Lair Boss class-Unique table.
  • Andariel's Loot Hoard Hoard chest opens after the kill once you spend the Greater Lair Key at the altar.
  • Resplendent Spark Tormented kill grants 1 Spark. 2 Sparks = 1 targeted Mythic craft at the Jeweler after the LoH update (down from 4).

Best Builds for Andariel

Whirlwind Barbarian

Stand inside her hitbox during open phase. Dust Devils stack on the boss + Effigies simultaneously.

Ball Lightning Sorcerer

Orbital ring inside her hitbox. Single-target DPS is competitive with the build's mob clear because all projectiles overlap on a stationary target.

Penetrating Shot Rogue

Cold Imbuement freezes the Effigies. Stand at range, line up shots through her + an Effigy.

Wing Strikes Paladin

Tankiest pick. Defiance Aura for the unstoppable window when she dashes; Holy Light Aura for sustain through Ground Seal stacks.

Frequently Asked Questions

Where do I fight Andariel in Lord of Hatred?

At the Hanged Man's Hall summoning altar in Kehjistan. Interact with the altar and spend 1 Greater Lair Key to open the encounter and the Hoard chest after the kill. The arena is a circular elevated platform.

How do I get Greater Lair Keys?

Greater Lair Keys are earned from endgame activities (Whisper rewards, Helltides, Nightmare Dungeons, and Lair Boss Hoards). Stockpiling Greater Lair Keys is the gate to chaining Greater Lair Boss runs.

How does the Effigy phase work?

Around 75% HP Andariel flies up and summons 3 Effigies of Anguish at fixed spawn points around the arena. While they exist, a rotating blood wave channels around the platform — getting hit by it stacks a heavy DoT. The objective is to burn down the Effigies as fast as possible. Strong AoE-cleave builds (Whirlwind Barb, Ball Lightning Sorc) clear all 3 in a single rotation. Single-target builds need to manually move clockwise faster than the wave rotates.

What is the difference between regular and Tormented Andariel?

Tormented Andariel costs additional Greater Lair Keys plus a Stygian Stone. The fight has the same mechanics with higher HP and damage values. Tormented kill drops Andariel's Loot Hoard (a bonus chest), 1 Resplendent Spark, and rolls on the full Mythic Unique loot table.

What is the stacking debuff and why does it matter?

Each unavoided mechanic — poison projectile, ground Seal step-in, fire beam clip — applies a stacking debuff that multiplies subsequent damage taken. By sub-25% the multiplier is large enough that even a tanky build dies to a single missed dodge. The fight punishes accumulated mistakes, not single hits — playing tight from the opening matters more than burst-healing recovery.

Can I solo Andariel as a fresh level-100 character?

Standard difficulty: yes, once you have your build's core uniques and a comfortable rotation. Tormented: requires solid gear (most likely a few Mythic crafts in) and either high mobility or a build that can stand inside her hitbox safely. For first-time runs, party with a tank player (Paladin or Druid) on Tormented if you don't have the gear yet.
